The following violation of very low safety significance (Green) was identified by Dominion and is a violation of NRC requirements which meets the criteria of the
NRC Enforcement Policy for being dispositioned as an
NCV.
Technical Specification 3.3.2 states, in part, that The Engineered Safety Features Actuation System instrumentation channels and interlocks shown in Table 3.3-3 shall be
operable with their Trip Setpoints set consistent with the values shown in the Nominal Trip Setpoint column of Table 3.3-4.
TS 3.3.3.4 states in part that these accident monitoring channels shall be
operable. Contrary to these requirements, all
main steam line steam generator pressure transmitters were reinstalled after maintenance using
gaskets that were not environmentally qualified for use in an accident environment, thereby rendering these transmitters
inoperable from January 17 through February 9, a condition prohibited by TS. Dominion identified the condition and immediately entered
TS 3.0.3. Dominion replaced the
gaskets and restored full
EQ qualification to all
main steam line pressure transmitters while complying with the action statements of
TS 3.0.3, and entered the issue into the corrective action program as condition report CR462222.
