| Title | Lack of Procedures for Monitoring the Performance of RHRSW Pump Motor Cooling Coils |
| Description | The inspectors identified a finding of very low safety significance and associated NCV of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, for the failure to prescribe a procedure for activities affecting quality. Specifically, the licensee did not develop procedures for monitoring the thermal performance of the residual heat removal service water pump motor upper thrust bearing oil cooling coils. This finding was entered into the licensees corrective action program (CAP) to generate procedures to collect monitoring data and to correlate to design conditions. The performance deficiency was determined to be more than minor because it was associated with the procedure quality and equipment performance attribute of the Mitigating Systems Cornerstone, and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. The finding screened as of very low safety significance (Green) because the finding was a qualification deficiency confirmed not to result in loss of operability or functionality. Specifically, the licensee performed a functionality evaluation and determined the most limiting cooler had sufficient margin. The inspectors determined that this finding had a cross-cutting aspect in the area of human performance because the licensee did not ensure supervisory and management oversight of work activities associated with the performance of residual heat removal service water pump motor cooler functional testing. Specifically, management did not ensure personnel developed procedures conforming to their Quality Assurance Program to be used when performing activities affecting quality. |
