Narrative

Report for LER 50-321/1981-114 On September 23rd whil e opera ting at 100% power, offgas levels as measured by the pretreatment monitor increased sharply.

Though no Technical Specification limits were exceeded it was decided to drop power Icvel to control the offges rates.

The de-rated operation continued until on October 9, the unit was t

shutdown to allow i dent i fi ca ti on and removal of the Icahing i

fuc1 assembli cs.

Sipping of all 560 fuel a ssembli cs identified 11 Reload 2 bundl es which contained f ail ed fuel rods.

Several of these rods were inspected and it was det ermined that there was the i

appearance of accelerated corrosion of the cladding surface.

On 10-29-81, a non-l ea k ing bundle from the same batch as the lenkers also exhibited siellar corrosion.

As the phenomena was not restricted to the leaker ass embli es it was decided that a condition of abnormal cladding degradntion existed.

Based on the inspection results of several of the Rc30ad 2 fuel assemblies, the affected bundl es have been determined to be unacceptable for use at this time and will be rep] aced.

A representative sampling of the other fuel types with si gnifi cant ex has been inspected and are acceptable for continued use.posure The affected fuel will be further st udied in an effort to determine the failure mechanism and the Commi s si on will be informed of any forthcoming conclusions.
