Axial Flux Difference - Outside the Target Band.

This report was initiated by Incident Reports81-505, 81-506,81-507, and 81-508.

CONDITIONS PRIOR TO OCCURRENCE:

12-19-81 Mode 1 - Rx Power 50% - Unit Load 580 MWe 12-21-81 Mode 1 - Rx Power 48% - Unit Load 560 MWe DESCRIPTION OF OCCURRENCE:

On four separate occasions, due to Load Rejection Testing, the Axial Flux Difference (AFD) shifted outside the +5% target band.

On December 19, 1981 at 0125, 0623, and 2220 hours0.0257 days <br />0.617 hours <br />0.00367 weeks <br />8.4471e-4 months <br />, during power reductions of 75% to 50%, 75% to 57%, and 85% to 50%, the AFD shifted outside the target band and Action Statement 3.2.1.a.2 d

was entered.

Again, at 2040 hours0.0236 days <br />0.567 hours <br />0.00337 weeks <br />7.7622e-4 months <br />, December 21, 1981, a power-reduction from 85% to 48% shifted the AFD outside the target.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

As power decreased, the AFD shifted upward.

At the same time, the rods going in pushed the flux downward, causing the AFD to shift out of the target band.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.2.1.a.2 requires:

Power operation may continue provided the indicated AFD has not been outside of the +5% target band for more than 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> penalty deviation cumulative during the previous 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, and the indicated AFD is within the limits shown on Figure 3.2-1.

Otherwise, reduce thermal power within 30 minutes.

CORRECTIVE ACTION

After the control rods were in and the flux stabilized, the AFD shifted back into the +5% target band.
