Radiation Monitor 2R12A - Inoperable.

i This report was initiated by Incident Report 81-496.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 98% - Unit Load 1140 MWe DESCRIPTION OF OCCURRENCE:

On December 12, 1981 the instrument setpoints on Radiation Monitor 2R12A failed high.

At 0717 hours0.0083 days <br />0.199 hours <br />0.00119 weeks <br />2.728185e-4 months <br /> 2R12A was declared inoperable and Action Statement 3.3.3.1 table 3.3-6 was entered.

This occurrence constituted operation in a degraded modo in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The 2R12A setpoints failed high from unknown causes.

Subsequent l

testing has failed to reveal a cause.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.1 table 3.3-6 requires:

With the number of channels operable less than required by the minimum channels operable requirement, comply with the Action Requirements of Specification 3.4.7.1.

Technical Specification 3.4.7.1 requires:

With only two of the required Leakage Detection Systems operable, operation may continue for up to 30 days pro-vided grab samples of the containment atmosphere are obtained and analyzed at least once per 24 houts.

CORRECTIVE ACTION

The monitor was removed and bench-tested satisfactorily.

The batteries were tested, and were fully charged, but were replaced as a precautionary measure.

The monitor was reinstalled, the setpoints reinstated, and it was tested satisfactorily.

At 1120 hours0.013 days <br />0.311 hours <br />0.00185 weeks <br />4.2616e-4 months <br /> 2R12A was declared operable and Action Statement 3.3.3.1 table 3.3-6 was terminated.

Subsequently, on January 6, 1982 the monitor functional test was satisfactorily performed.
