2C Vital Bus Undervoltage Relay - Inoperable.

This report was initiated ~by Incident Report 81-476.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe

DESCRIPTION OF OCCURRENCE:

On November 23, 1981, during the performance of the M3T maintenance

' surveillance test, it was discovered that the undervoltage relay on s

the 2C vital bus was actuating at 35% instead of the 70%. trip setpoint value.

The minimum required trip setpoint value is 65%.

The relay was placed in the tripped condition, and at 1557 hours0.018 days <br />0.433 hours <br />0.00257 weeks <br />5.924385e-4 months <br /> Action Statement 3.3.2.a Action 14 was entered, s

This occurrence constituted operation in a degraded mode in accord-ance with Technical Specification 6.9.1.9.a.

t DESIGNATION OF APPARENT CAUSE OF' OCCURRENCE:

The undervoltage-relay actuated at 35% instead of the 70% trip setpoint value because.it was mechanically sticking.

ANALYSIS OF OCCURRENCE:
