Salem Generating Station, Unit 2 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Containment Air Lock - Inoperable.

This report was initiated by Incident Reports81-468 and 81-491.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 91% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On two occa'sions, November 19 and December 7, 1981, while performing surveillance procedure SP (O) 4.6.1. 3 on the 130' elevation containment air lock, it was determined that the leakage rate exceeded the limit of 0.05 La at the design pressure of 47.0 PSIG as required by Technical Specification 3.6.1.3.b.

At 0100 and 1945 hours0.0225 days <br />0.54 hours <br />0.00322 weeks <br />7.400725e-4 months <br />, respectively, the air lock was declared inoperable and Action Statement 3.6.1.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The air lock did not seal properly due to misalignment of the inner door latch mechanism and failure of a latch bearing.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.6.1.3.a requires:

With one air lock door inoperable, maintain at least the operable air lock door closed and either restore the in-operable air lock door to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or lock the operable air lock door closed; otherwise, be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

LER 81-122/03L dORRECTIVE ACTION:

On November 19 the failed latch bearing was replaced; and on December 7 the door latch was properly readjusted.

Surveillance procedure SP(O) 4.6.1.3 was satisfactorily performed, and at 1542 and 2235 hours0.0259 days <br />0.621 hours <br />0.0037 weeks <br />8.504175e-4 months <br />, respectively, Action Statement 3.6.1.3.a was terminated.
