Individual Rod Position Indication - 2SB2 - Inoperable.

This report was initiated by Incident Reports81-466 and 81-467.

CONDITIONS PRIOR TO OCCURRENCE:

11-18-81 Mode 1 - Rx Power 86% - Unit Load 1000 MWe 11-19-81 Mode 1 - Rx Power 91% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On two occasions, November 18 and November 19, 1981, the Control Operator noticed that the Individual Rod Position Indicator (IRPI) and Control Rod Bank Demand Indicator for rod 2SB2 were in dis-agreement by greater than + 12 steps.

The Individual Rod Position Indicator was declared inoperable at 0815 and 0050 hours5.787037e-4 days <br />0.0139 hours <br />8.267196e-5 weeks <br />1.9025e-5 months <br />, respectively, and Action Statement 3.1.3.2.1.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

On November 18 the cause of the disagreement between the IRPI and the demand indicator was that the meter movement in the IRPI had drifted out of calibration.

The cause of the disagreement on November 19 was that the meter movement had failed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.1.a requires that in modes 1 and 2 with a maximum of one rod position indicator per bank inoperable either:

Determine the position of the non-indicating rod (s) indirectly by the movable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's position, or 2.

Reduce thermal power to less than 50% of rated thermal power within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

CORRECTIVE ACTION

On November 18 the meter movement in the IRPI for control rod 2SB2 was recalibrated, tested satisfactorily, and returned to service.

On November 19 the meter movement was replaced, recalibrated, tested satisfactorily, and returned to service.

The IRPI was declared operable, and at 1320 and 0415 hours0.0048 days <br />0.115 hours <br />6.861772e-4 weeks <br />1.579075e-4 months <br />, respectively, Action Statement 3.1.3.2.1.a was terminated.
