Solid State Protection System - Train B - Logic Failure.

This report was initiated by Incident Report 81-458.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

On November 13, 1981, while performing a surveillance test on the Solid State Protection System, Train B, the logic system for the reactor coolant loop flow failed the logic test.

At 1105 hours0.0128 days <br />0.307 hours <br />0.00183 weeks <br />4.204525e-4 months <br />, Train B was declared inoperable and Action Statements 3.3.1 Action 1 and 3.3.2.b Action 13 were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the logic test failure was the failure of an A303 universal circuit board.

Since this particular circuit was not called upon to perform its intended function prior to the surveillance test, it is assumed the circuit was inoperabir; however, the reactor was adequately protected by Train A.

CORRECTIVE ACTION

The failed A303 universal circuit board was replaced and tested satisfactorily.

Train B surveillance test was satisfactorily performed, and at 1118 hours0.0129 days <br />0.311 hours <br />0.00185 weeks <br />4.25399e-4 months <br /> Action Statements 3.3.1 Action 1 and 3.3.2.b Action 13 were terminated.

This was an electronic failure and not repetitive in nature.
