No. 22 Charging Pump - Inoperable.

This report was initiated by Incident Report 81-451 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 100% - Unit Load 1170 MWe DESCRIPTION OF OCCURRENCE:

On November 7, 1981, during normal operation, No. 22 Charging Pump tripped electrically on long-time overcurrent.

One restart was attempted and the pump again tripped.

The pump was declared inoperable and at 0527 hours0.0061 days <br />0.146 hours <br />8.713624e-4 weeks <br />2.005235e-4 months <br /> Action Statements 3.1.2.4 and 3.5.2.a were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of No. 22 Charging Pump failing to start was a broken impeller shaft in the pump.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.1.2.4 requires:

With only one charging pump operable, restore at least twa charging pumps to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least hot standby and borated to a shutdown margin equivalent to at least 1%

Delta K/K at 200 degrees Fahrenheit within the next 7 days or be in cold shutdown within the next 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

Technical Specification 3.4.2.a requires:

With one ECCS subsystem inoperable, restore the subsystem to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in het shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

On November 9, 1981, permission was obtained from ARC to extend the above action statements for 48 additional nouts in order to complete repairs.

CORRECTIVE ACTION

A new rotating assembly was installed in the pump and surveillance procedure 4.0.5(P) was performed satisfactorily.

The charging pump was declared operable, returned to service, and Action Statements 3.1.2.4 and 3.5.2.a were terminated at 2330 hours0.027 days <br />0.647 hours <br />0.00385 weeks <br />8.86565e-4 months <br />, November 10, 1981.

A full flow test will be performed the next time the unit is in mode 5.
