No. 21 Steam Generator Steam Flow Channel 2 - inoperable This report was initiated by incident report 81-349.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 25% - Unit Load 290 MWe DESCRIPTION OF OCCURRENCE:

On August 29, 1981 the control operator noticed that No. 21 steam generator steam flow channel 2 had failed low.

At 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> action statements 3.3.1 action 7 and 3.3.2.b action 14 was entered.

in accordance with technical specification 6.9.1.9.b.

l DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The No. 21 steam flow transmitter had failed.

  • LER 81-96/03L CORRECTIVE ACTION:

No. 21 steam generator steam flow channel 2 was placed in the tripped condition within one hour.

The steam flow transmitter was removed, replaced, and calibrated.

At 0300, August 30, 1981 the channel was tested satisfactorily and returned to service, and action statements 3.3.1 action 7 and 3.3.2.b action 14 were terminated.
