CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 90% - Unit Load 980 MWe DESCRIPTION OF OCCURRENCE:

As a result of the NRC's review of our " Cable Interaction, Study",

Conduit 2BDD-B(5") was identified as requiring fire wrap protection.

This item was listed in the NRC's report, tendered May 11, 1981, as table 2, item 4, category 4, criterion 14b, paragraph (a).

- The detailed wrapping was added to DCR 2EC-ll46, revision 15 on July 11, 1981, by the Site Engineer.

The revised document package 6

was forwarded to the Station Planning Engineer the same day via transmittal 359E.

Subsequently, the design information was incorporated into Catalytic work package OP-933.256-2201-3.

The package included wrapping the conduit in question at three different locations.

The diesel fuel oil storage tank room location was inadvertently overlooked.

This occurrence constituted operation in violation of Facility OperatingELicense DPR-75.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the noncompliance was that onefout of the three conduit

locations designated to be wrapped was[ inadvertently overlooked, and presumed completed.

ANALYSIS OF OCCURRENCE:

Without the required wrapping, a fire in the diesel fuel oil storage tank room could have resulted in the loss of ability to provide electrical power from two of the diesel generators if the redundant Automatic Suppression Systems failed to operate.

LER 81-85/0lT. CORRECTIVE ACTION:

A fire watch was immediately established.

At 2100 hours0.0243 days <br />0.583 hours <br />0.00347 weeks <br />7.9905e-4 months <br />, September 1, 1981, the wrapping of Conduit 2BDD-B(5") in the diesel fuel oil storage tank rconi, elevation 84', was completed and signed off to work package specifications.

Subsequently, a complete review of the cable wrapping design change package and other fire protection requirements of the Facility Operating License was conducted, and all required to date license commitments have been met.
