100' Elevation Containment Air Lock - Inoperable.

This report was initiated by Incident Reports81-318 and 81-321.

CONDITIONS PRIOR TO OCCURRENCE:

8-8-81 Mode 1 - Rx Power 77% - Unit Load 840 MWe 8-9-81 Mode 1 - Rx Power 75% - Unit Load 810 MWe DESCRIPTION OF OCCURRENCE:

On August 8 and August 9, 1981, while performing surveillance l

procedure SP (O) 4.6.1.3 on the 100 ' elevation containment air lock, i

it was determined that the leakage rate exceeded the limit of 0.05 La l

at the design pressure of 47.0 psig as required by technical specifi-cation 3.6.1.3.b.

At 2015 and 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />, on August 8 and August 9, respectively, the air lock was declared inoperable and action state-ment 3.6.1.3 was entered.

These occurrences constituted operation in a degraded mode in accordance l

with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

On August 8, it was determined that the outer door seal had come out of its channel, and was leaking.

On August 9, it was discovered that-a door cam roller had moved off its l

track, prever. ting a proper seal.

l ANALYSIS OF OCCURRENCE:

Technical specification 3.6.1.3 requires:

CORRECTIVE ACTION

On-August 8, the door seal was repaired and tested' satisfactorily.

JOn August 9, the door. cam roller was realigned.

The leak rate test was then performed satisfactorily.
