Solid State Protection System (S.S.P.S.) Train B - Incperable This report.was initiated by Incident Report 81-284.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power - 10 aamp - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On July 23, 1981, while performing the S.S.P.S. periodic test, SP (O) 4. 3. 2.1. l (e-b), the operator discovered that logic A switch in position 1 did not function properly in mode blocks inhibit and not inhibited.

Switch position 1 tests the loss of flow loop 1 circuitry.

S.S P.S. train B was declared inoperable and Limiting Condition For Operation (L.C.O.)

3.3.1, table 3.3-1, action 1 was entered at 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br /> on July 23, 1981.

This occurrence constituted operation in a degreded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

Universal board serial No. 0156 had failed.

ANALYSIS OF OCCURRENCE L.C.O.

3.3.1, table 3.3-1, action 1 states:

With the number of channels operable one less than required by the minimum channels operable requirement, be in hot standby within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

CORRECTIVE ACTION

Universal board serial No. 0156 was replaced in kind by universal board serial No. 0220.
