Reactor Trip Instrumentation - Engineered Safety Feature Actuation System No. 24 Steam Generator Steam ? low Channel 1 - Inoperable.

This report was initiated by Incident Report 81-267 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 48% - Unit Load 440 MWe DESCP.IPTION OF OCCURRENCE:

On July 21, 1981, during a routine channel check, the operator noticed that No. 24 steam generator steam flow channel 1 was reading 20% higher than the other channel.

The channel was declared inoperable and the channel bistable was placed in the tripped condition within one hour.

Limiting condition for oper-ation (L.C.0) 3.3.1, table 3.3.1, action 7 and L.C.O 3.3.2, table 3.3-3, action 14 were entered at 1020 hours0.0118 days <br />0.283 hours <br />0.00169 weeks <br />3.8811e-4 months <br /> on July 21, 1981.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Multiplier 2FM-542B was found to be high from its low end sctpoint causing the erroneous readings.

ANALYSIS OF OCCURRENCE:

3.3.1, table 3.3.1, action 7 and L.C.O 3.3.2, table 3.3-3, action 14 both state that:

With the number of operable channels one less than the total number of channels, power operation may proceed until performance of the next required channel funct-i ional test provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

Multiplier 2FM-542B was adjusted to its correct reading value.

The channel was restored to operable status and was reading correctly.
