Individual Rod Position Indication - 2D3 - Inoperable.

This report was initiated by Incident Report 81-266.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 48% - Unit Load 450 MWe DESCRIPTION OF OCCURRENCE:

On July 20, 1981, the control operator noticed that the individual rod position indicator and control rod bank demand indicator for rod 2D3 were in disagreement by greater then + 12 steps.

The individual rod position indicator was declarel inoperable at 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> and action statement 3.1.3.2.1 was enteted.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF AFPARENT CAUSE OF OCCURRENCE:

Equipment failure. The individual rod position indicator (IRPI) for control rod 2D3 was not in agreement with the control rod bank demand j

position due to drift in the calibration of the IRPI and the signal conditioning module.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.1.3.2.1 requires that in modes 1 and 2 with l

a maximum of one rod position indicator per bank. inoperable either:

Determine the position of the non-indicating rod (s) indirectly by the movable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod's position, or 2.

Reduce thermal power to less than 50% of rated thermal power within 8ahours.

CORRECTIVE ACTION

The IRPI and signal conditioning module for control rod 2D3 were recalibrated satisfactorily and returned to service.

Action state-ment 3.1.3.2.1 was terminated at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br />, July 21, 1981.
