Reactor Trip System Instrumentation No. 23 Steem Generator Feed Flow Channel 2 - Inoperable This report was initiated by Incident Report 81-259 CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 22% - Unit Load 120 MWe DESCRIPTION OF OCCURRENCE:

i 0, July 16, 1981, during routine channel checks, the control room operator noticed that No. 23 steam generator feed flow channel 2 6

had failed low.

The channel was declared inoperable, technical specification LCO 3.3.1, action No. 7, was entered at 0935 hours0.0108 days <br />0.26 hours <br />0.00155 weeks <br />3.557675e-4 months <br />, un July 16, 1981, and the channel was placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

1 The occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENFE:

Equipment failure.

No. 23 steam generator feed flow channel 2 meter had failed.

ANALYSIS OF OCCURRENCE:

Action statement 3.3.1.7 requires:

With the number of operable channels less than the total number of channels, power operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripced condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION

A spare meter was calibrated, installed and tested with satisfactory results.

The channel was declared operable and returned to service.
