Individual Rod Position Indication - ISB2 - Inoperable.

This report was initiated by Incident Report 81-205.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

During startup on June 23, 1981, the control operator noticed that the individual rod position indicator for rod ISB2 was in disagree-ment with the shutdown rod bank demand indicator by greater than

+ 12 steps.

The individual rod position indicator was declared Inoperable at 0624 hours0.00722 days <br />0.173 hours <br />0.00103 weeks <br />2.37432e-4 months <br /> and action statement 3.1.3.2.2 was entered.

This occurrence constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

The individual rod position indicator for shutdown rod ISB2 was not in agreement with the shutdown rod bank demand indi-cator due to failure of the amplifier board in the indicator.

ANALYSIS OF OCCURRENCE:

Technical specification 3.1.3.2.2 requires:

When one rod position indicator is not within + 12 steps of the rod bank demand irdicator, the reactor trip system breakers be opened immediately.

CORRECTIVE ACTION

The reactor trip breakers were opened immediately.

The amplifier board in the individual rod position indicator for shutdown rod ISB2 was replaced in kind.

The indicator was recalibrated satisfactorily and returned to service.

Action statement 3.1.3.2.2 was terminated at 1011 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.846855e-4 months <br />, June 23, 1981.
