Radiation Monitoring System - Containment Particulate Channel -

2RllA - Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

During startup operations, at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, June 8, 1981, the paner filter drive for the Containment Particulate Channel 2RllA failed.

The channel was declared inoperable and Action Statements 3.3.3.1 and 3.4.7.1 were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

Investigation of this occurrence disclosed the rachet arm link on the filter drive mechanism had failed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.1 requires compliance with the action requirements of 3.4.7.1 which requires that with only two leakage systems operable, operation may continue for up to 30 days provided grab samples of the containment atmosphere are obtained and analyzed at least once per 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> when the required gaseous and/or particulate radioactivity monitoring system is inoperable.

CORRECTIVE ACTION

The rachet arm link was replaced in kind.

Channel ".RllA was tested satisfactorily and returned to an operable status.

Action Statements 3.3.3.1 and 3.4.7.1 were terminated at 1540 hours0.0178 days <br />0.428 hours <br />0.00255 weeks <br />5.8597e-4 months <br />, June 8, 1981.
