Movable Control Assemblies - Rod 2D4 Dropped CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

Action Statement 3.1.3.lc was entered at 0840 on May 20, 1981 when Rod 2D4 dropped.

Emergency Instruction I.4.8 - Rod Control System Malfunction Part IV DropDed Rod - was performed unsuccessfully.

At 0940 a manual Reactor Trip was initiated.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure.

Subsequent investigation disclosed an open circuit in the CRDM connector at the reactor vessel head.

The connector pin failed due to corrosion within the connector.

A random sampling of ceveral other connectors disclosed no other cases of corrosion.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.1.3.lc requires that with one full length rod inopetable or misaligned from the group step counter demand height by more than i 12 steps (indicated position), power operation may continue provided that within one hour the rod is restored to operable status Uithin the alignment requirements.

CORRECTIVE ACTION

The connector pin that had failed was replaced in kind and a functional test was performed satisfactorily.

Action Statement 3.1.3.lc was terminated at 0940 hours0.0109 days <br />0.261 hours <br />0.00155 weeks <br />3.5767e-4 months <br /> on May 20, 1981.

FAILURE DATA:

Crouse - Hinds Connector pin was replaced in kind.
