February 28, 1982 Identification of Occurrence:

Failure to have two operable Makeup Punps contrary to Technical Specification 3.1.2.4.1.

Conditions Prior to Occurrence:

Mode 3 hot standby (0%).

Description of Occurrence:

At 1800, during routine inspection, a leak was discovered on the pressure side of Makeup Valve 292, (MUV-292) in the " heat affected" area of the weld. MUV-292 is the recirculation line vent valve for Makeup Pump 1A. Makeup Pump 1A was secured, and Makeup Punp 1C was started. Maintenance was initiated, and operability was restored at 1800 on 3/1/82.

Designation of Apparent Cause:

The apparent cause of this event is attrilyated to vibration.

Analysis of Occurrence:

The plant was in hot standby at the time of discovery, and there was no unplanned release. There was no effect upon the health or safety of the general public.

Corrective Action

The weld was replaced and nore support was added to Makeup o

Valve 292 to reduce vibration. Long term corrective actions are under engineering evaluation.

Failure Data:

This is the first reportable occurrence for this event and this is the first report under this Specification.
