| Description | 10 CFR 50, Appendix B, Criterion III, Design Control, requires, in part that design control measures shall provide for verifying or checking the adequacy of design, such as by the performance of design reviews or by the performance of a suitable testing program. Contrary to the above, the licensee failed to perform adequate post modification testing to evaluate the adequacy of a design modification to the RPS/ES system and allowed an improper wiring configuration of the Unit 1 Nuclear Instrumentation (NI) Power Range detector cables. The licensee failed to follow their Modification Test Plan to verify that the Flux/Flow/Imbalance function for the Power Range NIs would work properly. This finding was not greater than very low safety significance (Green) because the RPS could have still fulfilled its safety function with the channels inoperable based on other trip signals available. The licensee entered this issue into their CAP as PIP O-11-7081. |
