On three separate occasions during the period from October 17 until

' November 3, 1981, four class one snubbers were out of service.

This is a degraded mode of LCO 4.3.10 and is reportable per Fort St.

Vrain Technical Specification AC 7.5.2(b)2.

EVENT

DESCRIPTION

Event #1 On October 17, 1981,f during normal plant operation, at approximately 40% reactor power.and 118 MWe, plant maintenance personnel. found by field observation that vent stack snubber VSS-111 was low on oil and extended to the limit of its travel.

This made the snubber inoperable.

The snubber was removed from service, repaired and adjusted, then tested and returned to service on October 19 at 1740 hours0.0201 days <br />0.483 hours <br />0.00288 weeks <br />6.6207e-4 months <br />.

This single snubber failure does -not affect the surveillance interval.

Event #2 On October 30, 1981, boiler feed snubbers BFS-414E and BFS-417E were removed from service for support' structure modification.

They were returned to service the same day.

Event #3 On November 2, 1981, cold reheat snubber CRS-317 was removed from service for support structure modification.

It was returned to service November 3, 1981.

Events 2 and 3 do not constitute snubber failure and do not affect

,. the surveillance interval.

The support structure modifications were required by the continuing snubber seismic analysis program.

CAUSE

- DESCRIPTION:

Evant #1 The snubber was inoperable due to' thermal pipe expansion'which-extended the snubber until no further movement was possible.

Events #2 ano' #3 The snubber supports required additional welding per ongoing seismic analyses program.

CORRECTIVE ACTION:

Event #1 The snubber was removed from service, rebuilt, adjusted, and returned to service.

Event #2 and #3 Snubbers were removed from service.

The necessary welds were made and snubbers' returned to service.
