During performance of a scheduled Surveillance Test, the sample flow alam setpoints of three dewpoint moisture monitors were found outside the limits specified by LCO 4.4.1, Table 4.4-1, note (t). This event constitutes oper-ation in a degraded mode of LCO 4.4.1 and is reportable per Fort St. Vrain Technical Spa:ification AC 7.5.2(b)2.

EVENT

DESCRIPTION

While operating at 69% thermal power and 220 !N(e), plant instrument per-sonnel performed the primary c.oolant moisture instrumentation sample flow alarm functional test, SR 5.4.1.1.6e-M.

The test verifies that each of the eight dewpoint moisture monitors will alarm on low and high sample flow.

The limiting condition for operation LCO 4.4.1, Table 4.4-1, note (t) speci-l fies that a dewpoint moisture monitor shall not be considered operable unless the sa=ple flow is alarmed as follows:

l a) A fixed low sample flow alarm set at 1 sec/sec.

b) A fixed high sample flow alarm set at 75 sec/sec.

c) A variable icw sample flow alarm set at 1 to 50 sec/sec as a function of reactor power level.

During performance of the Surveillance Test, two fixed low sample flow alarm setpoints were found below the 1 sec/sec limit, two fixed high sample flow alarm setpoints were found above the 75 sec/see limit, and one variable low sample flow alarm setpoint was found below the 50 sec/sec limit required by the reactor power level at the time the surveillance was performed. Three dewpoint moisture monitors were involved - one high level monitor and one loop monitor in each loop. The actual setpoints found are shown for these three moisture montiors in Table 1.

CAUSE

DESCRIPTION:

The cause of this event was misadjustment of the alarm setpoints se some time in the past.

It has not been possible to determine the time the mis-adjustmsut occurred or what person was responsible.

CORRECTIVE ACTION:

The alarm setpoints were re-adjusted to the correct value and the Surveil-lance Test completed satisfactorily.

All Results personnel have been informed by memo that unauthori::ed ad-justments are not permitted, and that adjustments may caly be saade within the limits of LCO 4.4.1.

No further corrective action is anticipated or required.
