The 1D emergency diesel of 1B emergency generator set failed the weekly 50% load test.

EVENT

DESCRIPTION

p April 20, 1981 while op-rating at 50% thermal power and 170 W electrical power, the weekly emergency diesel generator 50% load test was conducted.

The surveillance for lA emergency generator set was completed satisfactorily.

The operator secured 1A emergency generator set and started testing on 1B emergency generator set.

Approximately 10 minutes before the two hour test period was completed, the operator checked the 1B emergency generator set and found it operr. Ling properly.

About five minutes later, the operator received a call from a security ' guard, who had observed an oil mist in the IB emergency diesel generator room.

The operator returned to the 13 emergency generator room and secured thc.

diesel generator set.

Upon examining the diesel engines, he discovered a fuel leak on //l injector of 1D diesel engine.

Maintenance personnel were called to repair the leaking fuel line and found the injector had failed and was jammed in the precombustion chamber.

The entire injector precombustion chamber was removed and replaced.

The a

emergency diesel generator set was retested and found satisfactory on April 23 1981, and'then returned to service.

The cause of this occurrence was a failed fuel injector.

The fuel injector and precombustion chamber were replaced.

The emergency generator set was retested and returned to service.

No further corrective action is anticipated or required.
