Upon discovery, movement of fuel assemblies was suspended in accordance with Technical Specification (TS) 3.7.12.A, until the door was closed. However, since the fuel handling area ventilation system was inoperable for approximately 10 minutes prior to discovery, the TS Required Action and associated Completion Time to immediately suspend movement of fuel assemblies was not met.

Palisades Nuclear Plant On October 1, 2007, with reactor refueling activities in progress, a door was discovered to be open in the boundary of the fuel handling area ventilation system [VG]. The open door constituted a bypass of the boundary, causing the fuel handling area ventilation system to be inoperable. The door is estimated to have been open for approximately 10 minutes.

CAUSE OF THE EVENT
During personnel transition through the doorway, the door stuck open due to interference with the floor and was not verified to re-close following passage.
CORRECTIVE ACTIONS
A description of the occurrence was communicated to site personnel, reinforcing the expectation to verify that doors close properly following passage. The sticking condition will be corrected.
SAFETY SIGNIFICANCE
The safety significance of this occurrence is considered to be minimal. The bounding fuel handling accident does not credit filtration of released fission products by the fuel handling area ventilation system. Analysis has shown that the offsite dose resulting from the bounding accident is within the guidelines of 10 CFR 100.
