Title | Low Pressure Alarms During Reduced Inventory |
Description | A finding of very low safety significance and associated NCV of 10 CFR 50.65a(4) was self-revealed for the failure to properly assess and manage risk when service water low pressure alarms were received during orange risk reduced inventory operations. The work control center authorized a non-critical service water valve to be stroked with the belief that the system was filled and vented thus precluding an impact on the service water system. However, that portion of the system had not been filled yet. As a result, opening the valve caused a pressure drop in the system. The licensee started a standby service water pump to restore pressure. The issue was also entered into the corrective action program. The inspectors determined the finding was more than minor based in-part on example 7g of IMC 0612, Appendix E, which describes a condition where a safety function is significantly degraded without sufficient compensation. Additionally, as described in IMC 0612 Appendix B, the issue is associated with the configuration control attribute and impacted the Initiating Events Cornerstone objective of limiting the likelihood of events that upset plant stability and challenge critical safety functions in that proper configuration control was not maintained over the shutdown equipment lineup. Utilizing IMC 0609 Appendix G, Shutdown Operations Significance Determination Process, the inspectors determined the issue was Green in Phase I screening since there was adequate mitigation capability and there was no loss of control. The finding had a cross-cutting aspect in the area of Human Performance, Work Control, because the licensee did not appropriately coordinate work activities by incorporating actions to address the impact of work on different job activities to assure plant performance. Specifically, the licensee failed to determine the current status of the service water system and did not evaluate potential impacts during a period of elevated plant risk. (H.3(b)) (Section 1R13) |
