PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE - PNO-I-09-001 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the Region I staff on this date.

UNUSUAL EVENT AND AUTOMATIC REACTOR SCRAM DUE TO A FIRE IN THE M1A MAIN TRANSFORMER At 9:56 p.m., on February 1, a main transformer failure and fire led to an automatic reactor scram and the subsequent declaration of an Unusual Event. Plant systems responded as expected to the scram with all control rods inserting. As designed, both isolation condensers actuated, two electro-magnetic relief valves cycled, and three of five reactor recirculation pumps tripped. The isolation condensers (IC) operated for two minutes and then were placed in standby. Operators stabilized the plant in hot shutdown and decay heat removal was accomplished by using the main feed and condensate, and turbine bypass valve systems. The station electrical system was aligned in its normal shutdown lineup with offsite power available.

The on-site brigade responded to the fire. After assessing the fire, off-site fire assistance was requested. Exelon declared an Unusual Event at 10:11 p.m. due to the fire not being extinguished within 15 minutes. The fire was extinguished at 10:27 p.m. by the on-site and off-site fire brigades, and the Unusual Event was terminated at 11:37 p.m. Exelon is evaluating plans to replace and/or repair this transformer. This transformer was placed in service in December 2008 after the previous transformer failed. (See PNO-I-08-013)

The acting Senior Resident Inspector responded to the site and assessed licensee actions throughout the night.

The State of New Jersey has been informed. Region I Public Affairs is prepared to respond to media inquiries. An update to this PN is planned.

The facts of this PN have been verified with Exelon as of February 2, 2009, at 10:45 a.m.
