| Description | Technical Specification 6.8.1 states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures as recommended in NRC Regulatory Guide 1.33, Appendix A, Revision 2, February 1978. NRC Regulatory Guide 1.33, Appendix A, Section 8.b requires procedures for surveillance tests. Limerick Unit 2 ST-6-107-590-2, Daily Surveillance Log/OPCONS 1, 2, 3, Table 2, Revision 128, contained the redundant reactivity control system TS surveillance channel checks for the reactor pressure vessel pressure instruments associated with the Anticipated Transient Without Scram Recirculation Pump Trip System Instrumentation. Contrary to TS 6.8.1, ST-6-107- 590-2 acceptance criteria for channel checks associated with these instruments were not adequately established. This resulted in the Unit 2, Division II Anticipated Transient Without Scram Recirculation Pump Trip System Instrumentation being inoperable due to instrument drift from September 8, 2011 until May 10, 2012 which is longer than allowed by TS. Exelon entered this issue into the CAP as IR 1365093. The inspectors determined that the finding was of very low safety significance (Green) in accordance with Section C of Exhibit 2 of NRC IMC 0609, Appendix A, The Significance Determination Process for Findings At-Power, because the finding did not affect both a single reactor protection system trip signal to initiate a reactor scram and the function of other redundant trips or diverse methods of reactor shutdown. |
