| Title | Failure to Adequately Evaluate the Suitability of NONSAFETY-RELATED Gaskets, O-RINGS, and Seals Installed in SAFETY-RELATED Equipment and to Identify Extent of the Condition |
| Description | The team identified a non-cited violation of 10 CFR Part 50, Appendix B, Criterion III, Design Control, for the licensees failure to evaluate the suitability of Non-safety-related gaskets, o-rings, and seals installed in safety-related components. These non-safety-related parts were originally installed due to erroneous Safety Classification Assessments. After determining that the parts were inappropriate in safety-related joints, the licensee failed to promptly correct the condition and failed to fully identify which components were affected. The licensee entered this finding in its corrective action program as condition report 53456. The failure of the licensee to evaluate the suitability of the specific non-safety-related material installed in safety-related equipment and to determine the extent to which this condition existed was a performance deficiency. This performance deficiency was more than minor because it affected the design control attribute of the mitigating systems cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the inadequate evaluation of non-safety-related gaskets, o-rings, and seals installed in safety-related equipment adversely affected the reliability of the affected systems. Using Manual Chapter 0609.04, Phase 1 - Initial Screening and Characterization of Findings, the team determined that the finding was of very low safety significance (Green) because the finding was a design or qualification deficiency confirmed not to result in loss of operability or functionality. This performance deficiency had a cross-cutting aspect in the corrective action program component of the problem identification and resolution cross-cutting area because the licensee did not take appropriate corrective actions to address safety issues and adverse trends in a timely manner, commensurate with their safety significance and complexity |
