Cause of event

was relay 1 6A-K63B fai 1ing causing fuse 1 1 MIC-5A to blow.. Genera 1 Electric, coil size 55-1GZ, model CR12DA04002AA.

The relay and fuse were replaced.

No recurrence control required, failure was within the expected life of the relay.

Relay 16A-K633 overheated and failed blowing fuse 11 MIC-5A.

This caused primary containment isolation system group 6 valves, drywell compressor valves and drywell leak detection valves to fail closed.

Also caused control room isolation division I & II; containment atmospheric monitoring division I & II and refuel floor vent monitor 3-90-140A to fail in alarm condition.

Analysis of Occur rence:

There was no damage to plant equipment.

There was no activity release, no personnel exposure or injury and no danger to health or safety of the public.

Failures were in the conservative direction.

Corrective Action

The relay and fuse were replaced.
