21 March 1979 Identification of Occurrence:

One independent ECCS subsystem inoperable contrary to Technical Specification 3.5.2.

Conditions Prior to Occurrence:

Mode 1 power operation (100%)

Description of Occurrence:

At 2200, while performing maintenance on MUV-6, it was discovered that makeup pump 3-C discharge stopcheck valve MUV-2 was improperly positioned. MUV-2 was found at 2-3/4 turns open as opposed to the proper setting of 3-1/4 turns open.

MUV-2 was reset to its proper position and was returned to service at 2220.

Designation of Apparent Cause:

The cause of this event is unknown.

Analysis of Occurrence:

No hazard to the plant or general public as the correct throttled valve position was restored within 20 minutes.

Corrective Action

MUV-2 was reset to its proper throttle setting and returned to service at 2220. Makeup and Purification System Procedure OP-402 has been revised to include throttle settings for makeup valves 2, 6, and 10 in accordan'ce with Test Procedure 7-1-202-3.

This revision is made to comply with Technical Specification Amendment 17.
