1 August 1979 Identification of Occurrence:

Reactor Building isolation valves CAV-4 and CAV-6 inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 1 power operation (25%)

Description of Occurrence:

At 0200, it was reported that Reactor Building 'A' OTSG sample line isolation valves, CAV-4 and CAV-6, would not close when securing from continuous blowdown.

Manual isolation valves CAV-318, 321, 14d and 149 were closed and CAV-4 and CAV-6 were deactivated in the closed position as per the action statement requirements of Technical Specification 3.6.3.1.

Maintenance restored operability and CAV-4 and CAV-6 were returned to service.

Designation of Apparent Cause:

The cause of this event is attributed to CAV-4 being stuck on its backseat and CAV-6 having insufficient lubrication.

Analysis of Occurrence:

No effect upon the plant or general public as manual isolation valves CAV 148, 149. 321 and 318 were closed.

Corrective Action

CAV-4 was freed from its backseat and CAV-6 was lubricated.

Both valves were returned to service after satisfactory completion of Quarterly Cycling of Valves Procedure SP-370.

Request for Engineering Information 79-7-3 has been written to evaluate this event and provide solutions to prevent recurrence.

Failure Data:

This is the sixth occurrence of this type reported.
