Post replacement testing is scheduled and will be completed prior to returning AHFL-2A to service.

Damage to Auxildt ry Building Ventilation Exhaust Filter in' excess of two-tb>usand ($2,000) dollars.

Conditions Prior to Occurrence:

Mode 5, cold shutdown.

Description of Occurrence:

At 1145 it was discovered that damage to Auxiliary Building Ventilation Exhaust Filter AHFL-2A had occurred as a result of an actuation of the Fire Service Deluge System.

Estimated cost of filter replacement is two-thousand seven hundred fifty dollars

($2,750).

Further investigation revealed that the deluge trip mechanism was actuated as a result of a water hammer in the system caused by the inadvertent initiation of the Fire Service Water Curtain System while attempting to reset a fire service panel alarm.

Designation of Apparent Cause:

The cause of this event is attributed to personnel error.

i Analysis of Occurrence:

No threat to the health and safety of the plant staff or public.

Auxiliary Building exhaust ventilation was shifted to AHFL-2B at 1300.

Samples of water washed through the filters into the Auxiliary Building drain system revealed concentrations of activity less than the limits imposed by 10 CFR 20.

Corrective Action

A review of this event has been conducted with the responsible section to prevent recurrence.

In addition, protective covers will be installed over the Fire Service System actuation push-buttons on the Fire Service System alarm panel.
