6.9.1.13.b Reported Under Technical Specification 6.9.1.13.b Date of Occurrence: 5/16/80 Time of Occurrence: 0800 EDT Unit 1 Identification and Description of Occurrence Containment isolation valves FCV 1-7, -14, -25, and -32 are not protected against a steam jet resulting from a c ack in loop 1 main steam riser.

These valves could be damaged by this steam jet.

Conditions Prior to Occurrence Reactor in mode 5.

Action Specified in the Technical. Specifi' cation Surveillance Requirements Met Due to Inoperable Equipment None required in mode 5.

j Apparent Cause of Occurrence Division of Engineering Design misinterpreted information provided by i,

Westinghouse and deleted the supports and protective devices.

Analysis of Occurrence If a crack were to occur in main steam riser, the steam jet impinging on the blowdown lines could cause all four valves to be damaged.

Corrective Action

The deleted supports and protective devices will be added by ECN 5285.
