2 October 1980 Identification of Occurrence:

Failure to have containment isolation valves operable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 1 power operation (100%)

Description of Occurrence:

At 2323 after pumping down the Reactor Coolant Drain Tank, WDV-94, RC Drain Tank Isolation Valve failed to close from the Control Room.

Manual valves WDV-808 and 840 were used to isolate the tank, and maintenance was initiated. Operability was restored at 1120 on 3 October 1980.

Designation of Apparent Cause:

The cause of this event is attributed to an inadequate setting on the. torque switch.

Analysis of Occurrence:

There was no effect upon the general public health or safety.

Corrective Action

The torque switch setting was adjusted and a functional test was performed. A reinspection of the torque switch setting will be performed in three (3) months.
