Failure to have containment. isolation valves operable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

l Mode 1 power operation (100%).

I Description of Occurrence:

At 1837 on November 16, 1980 after sampling the pressurizer water space, it was discovered that containment isolation valve CAV-3 would not close. CAV-2 was closed and deactivated j

and maintenance was initiated. Operability was restored at 0115 November 17, 1980. At 0950 November 18,1980 CAV-3 again failed to Close. CAV-2 was again closed and deactivated and additional maintenance was initiated. Operability was i

restored at 1620 November 19, 1980.

Designation of Apparent Cause:

The cause of these events is attributed to inadequate closing

torque, i

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action

The valve stem for CAV-3 was lubricated after the first event and the functional test was-satisfactory. After the second event, the torque switch setting was increased. No further I

corrective action required.

Failure Data:

These are the second and third occurrences for CAV-3,and this is the twenty-eighth event reported under this Specification.
