< 1 REPORT OF INTERVIEW WITH -

GREGORY J. JAROSZ On June 28, 1994, Gregory J. JAROSZ, Product Manager, Transco Products, 55 East Jackson Blvd., Chicago, Illinois, was interview by NRC:01 Senior Investigator Richard C. Paul. JAROSZ was interviewed at NRC:RIII offices in Lisle, Illinois. JAROSZ had been previously interviewed on January 28, 1994, regarding this investigation.

JAROSZ was shown a Dow Corning memorandum addressed to Dow Corning Power Applicators, dated November 12, 1984, which discusses a slight modification to the formulation of the Dow Corning 3-6548 Silicone RTV foam. JAR 05Z said he worked for Transco in 1984, and Transco was a Dow Corning Power Applicator in November 1984, but he does not recall seeing this memorandum. JAROSZ said he searched through Transco's documents in preparation for the January 1994 NRC:01 interview and did not find this memorandum.

JAROSZ said he determined the dates of manufacture for the Dow Corning 3-6548 foam used in the 1985 fire penetration seal tests conducted for Northeast Utilities (Millstone 3). JAROSZ said the seals for the first two tests which failed were made up of Dow Corning foam manufactured before the effective date of the formula modification. The last three tests were conducted with foam made up of both pre and post formulation modification foam. One of these tests failed, but the other two passed. JAROSZ provided OI with a test matrix describing this. ,

INVESTIGATORS NOTE: Dow Corning 3-6548 silicone foam is a two part product which is mixed to form the penetration seals. For the last three tests, the Part A was manufactured before the formulation modification, and the Part B was manufactured after the modification.

JAROSZ said he spoke with Dow Corning on what procedures Dow Corning follows when they change the formula on one of their products. JAROSZ said he was told by Dow Corning that once a formula is changed it is given a new product number / description. JAROSZ commented that based on the Dow Corning memorandum of November 1984, Dow Corning had not followed their procedures by failing to give the silicone foam a new product number / description.

'JAROSZ remarked that at Millstone 3, Transco recommended that a gypsum product be used for the penetration seals, but Stone & Webster (S&W), the Millstone 3 architect / engineer, insisted on the use of Dow Corning silicone foam. Accord-ing to JAROSZ, S&W had just finished the Shoreham nuclear plant and had used silicone foam penetration seals installed by SISCO. JAROSZ, who said he had previously worked for BISCO in the early 1970s, said BISCO used another name for the seal material which they installed, but, in fact, this material was Dow Corning 3-6548 silicone foam. JAROSZ commented that he felt S&W was trying to get rid of Transco on the seal contract at Millstone. JAROSZ said that a meeting was held between Northeast Utilities, S&W, and Transco at which time it was decided to have Transco install silicone foam seals. Transco then -

conducted the qualification tests which initially resulted in failures of 10 l and 12 inch undammed silicone foam penetration seals. The foam did not I  ;

qualify until a one inch damming board was added to the seal. I .I '

, l JAROSI said S&W was surprised by the failures of the Transco tests. JAROSZ felt their surprise was because at Shoreham, BISCO was installing nine inch undammed seals. JAROSZ commented that the performance of a penetration seal l

in.a fire test is dependent on the configuration being tested and the instal-lation procedures used to install the material. JAROSZ acknowledged that '

BISCO installers are the same craft personnel used by other seal applicators, including Transco, for the installation of silicone foam penetrations seals.

JAROSZ said he is unaware of the testing conducted by BISCO to qualify their nine inch penetration seal.
