,- HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING APRIL 17, 1987 Seabrook Unit 1 On April 9,1987, the Commission completed its review of ALAB 853. THe issue raised in ALAB 853 is whether any license can be issued prior to the submittal of offsite emergency plans. The Comission decided (by 3-2) not to affirm the Appeal Board's decision. The Commission stated: "In the special circumstances of this case our. judgement is that sound policy favors requiring the filing of a State, local, or utility plan before any operating license is issued, including a license confined to fuel loading or low-power testing." However, the Corsnission decided not to take action with respect to the outstanding Seabrook, Unit 1 license for fuel loading and precriticality testing.

The Comission, in noting an April 8,1987 PSNH filing of an offsite plan, indicated that the " plan makes possible at least a sumary review of the type we performed in Shoreham, to determine whether adequate emergency planning is at least in the realm of the possible." The Comission indicated that views of the parites, on the April 8,1987 PSNH motion to lift the stay on the low power license are due from the parties by April 28, 1987 and the NRC staff by May 1, 1987.

Shoreham On April 14, 1987, theLongIslandLightingCompany(LILCO)fileda Motion for Expedited Connission Consideration of a Request for Authorization to increase power to 25%. The request is being made pursuantto10CFRSection50.47(c),whichidentifiestheconditions underwhichanoperatinglicensemay(beissuedevenifsomeofthestandards 31ans are not set forth in S satisfied. The staff is planning an expedited review of tiis submittal from LILCO.

U.S. Uranium Enrichment In response to a 00E request, the Safeguards staff in coordination with Fuel Cycle staff is reviewing patential issues which should be considered in NRC licensing of a gaseous diffusion plant. This review is in response to questions raised on the privatization of the U.S. uranium enrichment enterprise.

On May 5, 6, and 7, NRC staff will visit Oak Ridge, TN to discuss and review DOE-contractor information developed through safety and environmental analysis of the gaseous diffusion plants at Paducah, Kentucky, and Portsmouth, Tennessee.

Staff will judge the scope and depth of supporting information, as well as the criteria used for design and analysis. The nature and condition of construc-tion and operational records will also be examined.

Cracked Stiffenina Rinas on UFg Cylinders NRC (RIV) has information that some 48-inch diameter UFc cylinders have stiffen-  !

ing rings fabricated from material which may not meet A5ME and ANSI Standards.

Radial cracks have been detected on some of these stif(ening rings. 00T regu-lations which become effective on June 30, 1987, will require UF cylinders to meet ASME and ANSI Standards. Loadingofaffectedcylindersshobidbediscon-tinued until the safety significance of the cracked rings has been evaluated and until it is clear that the cylinders can be transported under the new DOT rules. The staff is preparing a Bulletin to licensees on this subject.

Issuance of Renewal Facility Operating License for the University of New Mexico Research Reactor On March 24, 1987, the Office of Nuclear Reactor Regulation renewed Facility Operating License No. R-102 for the AGN-201M research reactor at the University of New Mexico (UNf1), in Albuquerque, New Mexico.

The AGN-201 reactor was transferred to the UNM form the University of California, Berkeley in August 1966. It was licensed for operation at UNM in September 1966. This renewal is for 20 years.

The AGN-201 is a small research reactor, designed to operate at a power level of 5 watts, and is used in experimental programs requiring a low neutron flux level. The inherent design features of this reactor offer l a high degree of safety, and the lower pcwer level precludes the l buildup of large amount of fission products.

Catawba Unit 2 ,

On April 13, 1987, Catawba Unit 2 proceeded to Mode 3 (Hot Standby) frcm full power because the Reactor Coolant System (RCS) unidentified leak rate was 1.8 gpm (TS limit is 1 gpm). Duke Power Company's personnel reduced the rate to less than 1 gpm by isolating a leaky valve on a RCS radiation monitor line. The Unit was back on line on April 14,1087, at about 5:50 p.m.

On March 23, 1987, the same Unit had a RCS leak rate in excess of I gpm. The cause of the leak was a leaky packing of a pressurizer PORV block valve. As a result, the Unit was shutdown from march 23 until April 8, 1987.

River Bend Station At about 3:15 p.m. (CDT) on April 13, 1987, the licensee implemented a software change to their process control computer while the reactor was operating at 100% rated power. This software change, provided by General Electric, was designed to affect balance of plant variables only.

This software modification apparently affected the ability of the computer to detect changes in thermal power levels as recirculation flow changed. The licensee estimates they exceeded their licensed thermal power level of 2894 MW for about four hours with a peak power level of about 101% of the license limit. The licensee began reducing power levels at about 7:00 p.m. when an operator suspected thermal power readouts were in conflict with other control room instrumentation.

River Bend Station received their full power license on November 20, 1985; went into commercial operation in June 16, 1986 and has been operating at 100% rated power for the past 89 days.

Region IV is following up closely on the licensee's root cause analysis of thie event is following up with General Electric.

Calvert Cliffs Unit 1 and 2 On April 14, 1907 Calvert Cliffs Unit I was in Mode 5 (cold shutdown) to correct environmental oualification deficiencies. Unit 2 was in Mode 6 (refueling).

At 1315 hours0.0152 days <br />0.365 hours <br />0.00217 weeks <br />5.003575e-4 months <br />, containment spray (CS) was inadvertently initiated in the Unit I containment through the #11 CS header. The CS continued for approximately 3 minutes before being secured. The licensee estimates that 4000 gallons of pH 5, borated water from the #11 refueling water tank (RWT) was sprayed into the Unit I containment.

APRIL 17, 1987 ENCLOSURE B

This inadvertent CS actuation was the result of, operator error and the ongoing correction of environmental qualification deficiencies.

No personnel injuries resulted from the spray actuation although several personnel were externally contaninated. These personnel were fully decontaminated by showering. ?!o internal contamination occurred.

No equipment damage was noted by the licensee but several terminal boxes were open for environmental qualification and consequently wetted by the spray. The licensee states that all terminal boxes and associated electrical corponents will be verified dry prior to reactor startup. Startup, which is scheduled for 4/18/87, is not expected to be delayed.

Shoreham On April 14, 1987, the Long Island Lighting Company (LILCO) filed a Motion for Expedited Commission Consideration of a Request for Authorization to increase power to 25%. The request is being made pursuant to 10 CFR Section 50.47(c), which identifies the conditions under which an operating license may be issued even if some of the standards set forth in Section 50.47(b) for the emergency plans are not satisfied. The staff is planning an expedited review of this submittal from LILCO.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending April 17, 1987 Second Annual State / Tribal Meeting NMSS is sponsoring the 2nd Annual Meeting of State and Tribal representatives in the High-Level Waste Program on June 9-10, 1987, at the Embassy Square Suites, 2000 N Street, NW, Washington DC. As part of the continuing and early exchange of data between NRC, DOE, states, Indian tribes, and other Federal agencies, NRC is holding this meeting to discuss various activities underway in our program. Also, included in the program will be NRC's approach to ident-ification and resolution of issues, status of NRC rulemakings, and demonstra-tion of the pilot program associated with the Licensing Support System.

U.S. Uranium Enrichment In response to a DOE request, the Safeguards staff in coordination with Fuel Cycle staff is reviewing potential issues which should be considered in NRC licensing of a gaseous diffusion plant. This review is in response to questions raised on the privatization of the U.S. uranium enrichment enterprise.

Turkey Point 4 Core Reverification by IAEA  !

l The inspection team from the IAEA successfully reverified the inventory in core and the spent fuel pool on April 9 and 10. The IAEA surveillance cameras will be in place until the core is closed and IAEA inspectors place seals on the core.

Meeting on Proposed Revisions to IAEA Transport Regulations On April 13, 1987, NRC representatives met with DOE and D0T staff to discuss and develop preliminary US positions on proposals from IAEA member states to review IAEA's transport safety regulations (Safety Series No. 6). The more significant proposals concerned shipments of plutonium by air, shipments of greater than Type A quantities of LSA materials, and the use of Type B(u) packages. United States views on these proposals will*be presented at an IAEA meeting in Vienna, Austria in June 1987.

2 Cracked Stiffening Rings on UFg Cylinders ,

NRC (RIV) has information that some 48-inch diameter UF g cylinders have stiffen-ing rings fabricated from material which may not meet A5ME and ANSI Standards.

Radial cracks have been detected on some of these stiffening rings. 00T regu-lations which become effective on June 30, 1987, will require UF g cylinders to meet ASME and ANSI Standards. Loading of affected cylinders should be discon-tinued until the safety significance of the cracked rings has been evaluated and until it is clear that the cylinders can be transported under the new D0T rules. The staff is preparing a Bulletin to licensees on this subject.

Convention on the Physical Protection of Nuclear Material The purpose of this convention is to provide for the establishment and maintenance of adequate physical security with respect to international shipment of significant quantities of source or special nuclear material. The convention entered into force on February 8,1987, the thirtieth day following the date on which the twenty-first country formally ratified it. On March 26, 1987, NRC amended Parts 40, 70, 73, and 110 to bring them into accord with the convention. The staff estimates the convention may require Commission involvement in approximately 1500 notifications of import, export, and l

transient shipments per year. SGTR staff has already responded to several export notifications and requests for information from licensees.

Kerr-McGee, West Chicago Representatives of Kerr-McGee Chemical Corporation and Kerr-McGee Corporation met with representatives of NMSS/FC on April 10, 1987. General aspects of the West Chicago site were discussed, including a brief site history and the status of current activities.

West Valley Demonstration Project (WVDP)

The vitrification system for the WVDP high-level waste has resumed cold testing af ter a four-month idle period. A 64-hour integrated vitrification process test was successfully completed. NRC staff observed a portion of the test run.

Dry Spent Fuel Storage A license amendment was issued to Carolina Power and Light for the 10 CFR Part 72 license authorizing dry spent fuel storage at its H. B. Robinson plant site. The amendment approves design changes to the internal fixtures of the stainless steel canister which will be emplaced in the horizontal concrete storage modules. The staff's evaluation confirmed acceptability of the re-quested modifications from the criticality safety and structural standpoints.

3 Status of Hearings:

Sequoyah Fuels, Gore, Oklahoma, Comprehensive Waste Disposal Plan Sequoyah Fuels Corporation submitted the Comprehensive Radiological Solid Waste Management Plan as required by Source Material License No. SUB-1010, Condition 21 on November 13, 1986. There has not been a schedule set for this proceed-ing. On March 25, 1987, Sequoyah Fuels Corporation petitioned for termination of the proceeding on the basis that no onsite disposal is now planned. The Presiding Officer has not yet ruled on the petition. The staff is not a party to the proceeding.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest

, Week Ending April 17, 1987 2D/3D Personnel from the Japan Atomic Energy Research Institute (JAERI) reported at

, the March 17, 1987 Instrumentation Review Meeting at INEL that most of the INEL instruments provided to the Japanese Slab Core Test Facility (SCTF) Core III worked satisfactorily. However, there were a few exceptions. Lower plenum turbine flow meters which were designed to measure core inlet flows did not produce good results because of the electromagnetic interference caused by the electrical heater rods. Some conductivity probes for measuring the liquid i distribution in the upper plenum and the downcomer were also unreliable. In

addition, three out of 12 turbine flowmeters in the upper plenum showed small oscillations before appreciable two-phase flows were detected. These problems will be looked into by INEL to detemine whether the data taken need

! corrections. A similar review meeting was held at LANL for ORNL instruments on

March 19-20, 1987. JAERI reported that almost all of the ORNL instruments also i performed satisfactorily. The only exception were downcomer string probes and one of the tie-plate drag bodies which appeared to touch the surrounding stationary part of the tie plate. They also reported that although instruments j functioned correctly, the concept of measuring the liquid film velocity on the

! surface of structural elemer.ts in the vessel and the velocity of two-phase i mixture in the core did not work because of poor coherence of signals obtained  !

j from two adjacent sampling points. l

! JAERI personnel reported that their effort for improving TRAC computer code

assessment efforts by the Japanese industry are about to start. They indicated i that they used special correlations for void fraction (Murao-Iguchi model) and

heat transfer (Murao-Sugimoto model) which were developed from their small scale test facility data and then confirmed that these correlations satisfactorily predicted the experimental data from large scale test a facilities; the Cylindrical Core Test Facility (CCTF) and the Slab Core Test j Facility (SCTF).

LANL indicated it also obtained good predictions of the CCTF data using a a

different set of void fraction and heat transfer correlations which were used in EPRI codes. LANL further noted that it used EPRI correlations because they cover a wider range of test conditions. LANL is now proceeding with developmental assessment of these new void distribution and heat transfer i

, models. These new models are planned to be released this June as part of TRAC-PF1/ MOD 2.

. 2 Fission Product Release From High Temperature Fuel One of the significant contributors to severe accident source term uncertainty for early containment failure accidents is in-vessel fission product release.

The Oak Ridge high tenperature fission product release program is being conducted to obtain realistic release data to reduce this uncertainty. The first of a series of tests, Test VI-3, has been conducted in a verticle furnace using irradiated fuel which was heated to 2700K using a heatup and cooldown rate of 0.3%. A number of technical problems were encountered which delayed this first test until now. Results of this test will be described in upcoming weekly reports. This test represents the highest temperature achieved to date in out-of-pile fission product release studies previously run in any laboratory.

National Academy of Sciences Review of Severe Accident Chemistry A meeting was held on April 10, 1987 involving NRC staff and staff from the National Research Council of the National Academy of Sciences to discuss details of a workshop to review existing research on high temperature chemical processes in severe nuclear accidents. The workshop, which is made possible by a grant from NRC, is scheduled for August 13-15, 1987 at Woods Hole, Massachusetts. The National Research Council staff reauested NRC's assistance on specifying topics and speakers for the workshop. NRC staff suggested three areas upon which to focus the review of severe accident chemistry. Several general overview papers were also proposed to acquaint the scientists at the workshop with severe accident conditions and relevant chemistry phenomena.

Both U.S. and foreign scientists were nominated as speakers for the workshop.

. Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending April 17, 1987

1. Incident Response A meeting was held in Region III on April 15, 1987 to discuss plans for the Zion Full Field Exercise (FFE) with the licensee and federal and state representatives.

Following that meeting, AE0D and NRR representatives for Reactor Safety and Protective Measures Teams met with the Illinois Department of Nuclear Safety in Springfield, Illinois for a briefing on the REAC facility on April 16, 1987. A potential problem was identified regarding communication of data uniquely obtained by the Illinois Department of Nuclear Safety.

Provisions for obtaining the data from Illinois are being made and arrangements with other states will be examined to assure data uniquely available to each state is disseminated to NRC and the licensee.

The NRC is coordinating an effort to develop a videotape of the FFE at Zion. DOE, FEMA and Commonwealth Edison will film for their own needs and to support a script developed by NRC State Programs. The footage will be edited into a 30 minute presentation by FEMA or an NRC contractor.

2. Technical Training Center Several members of the TCC received hands on training at the Westinghouse SNUPPS-2 simulator in Monroeville, Pennsylvania from April 6-10, 1987.

The Westinghouse SNUPPS simulator, to be relocated to the TTC in Chattanooga, Tennessee, is being upgraded to the functional equivalent of

the existing SNUPPS-2 machine. This PWR instructor training was included as part of the Westinghouse simulator contract and will allow the PWR Technology Branch to provide 6 simulator courses using this simulator over the next 3 months and prior to its availability at the TTC. Initial work at the TTC to prepare the space for the SNUPPS simulator has begun.

This space was obtained by modification of an existing TTC contract. The SNUPPS simulator is expected to be operational at the TTC in July 1987.

The systems information in the PWR Technology Manual (Westinghouse. design) is being revised and converted from Sequoyah Nuclear Plant specifics to match the SNUPPS design which will be present on the Westinghouse SNUPPS simulator being relocated to the TTC. This is similar to the effort conducted by the TTC's Boiling Water Reactor (BWR) Technology Branch

within the last 18 months to convert the BWR teaching base from the l Browns Ferry Nuclear Plant design to a BWR-6 base.

I The Fundamentals of Inspection Course (F0IC) Manual has been forwarded for technical editing. The manual represents the culmination of a significant -

effort on the part of several NRC regional and program office personnel to revise and update existing subject material and create new material. The first F0IC using all revised materials will be coordinated by the TTC and will take place in the middle of the calendar year.

3. Reactor Operations Analysis j AE0D has initiated a study of the boric acid events at Turkey Point 3 and ANO-1. The study is expected to be completed in three months.

A MOU is being coordinated between NRC and INPO regarding the Performance l Indicator program information sharing.

4 i As a result of the Diablo Canyon loss of decay heat removal event, AEOD is l reviewing existing information including a previous case study, Informatiort Notices and INP0 evaluations to review the risk associated with these events

4. Trends and Patterns .

The proposed bulletin resulting from the Surry pipe break was sent to CRGR on Thursday with agreement of NRR since it is a continuity item from IE.

The bulletin would request information from licensees regarding their programs for detecting pipe wall thinning in carbon steel piping. It would allow for industry initiatives to develop the method of inspection and the basis for locations to be inspected.
5. CRGR The CRGR met April 17, 1987 to review a proposed generic letter on Appendix J leak rate testing calculation methods and RG E005-6 on Battery

, Qualifications. The CRGR recommended resolution of the leak rate testing by a minor rule change. The CRGR also recommended adding considerations of backfitting the Regulatory Guide for replacement batteries and requesting comments on the adequacy of thermal aging test methodology.

1. A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $?,000 was issued April 14, 1987 to Newport News Shipbuilding and Dry Dock Company, Newport News, VA. This action is based on an overexposure during normal operations and during the retrieval of a 10.2 curie iridium-192 radiography source. (EN 87-25A)
2. An Order Imposing a Civil Penalty in the amount of $50,000 was issued April 14, 1987 to Duke Power Company (Catawba, Unit 2). This action was based on violations of NRC requirements associated with the June 27, 1986 depressurization event during a loss of control room test. The licensee responded in a letter dated December 12, 1986. After careful consideration of the licensee's response, the staff concluded that with the exception of one violation, the violations did occur as set forth in the original Notice of Violation and Proposed Imposition of Civil Penalty. 3. A Notice of Violation ard Proposed Imposition of Civil Penalty in the amcunt of $2,500 was issued April 15, 1987 to Cleveland Clinic Foundation, Cleveland, Ohio. This action is based on (1) the failure to make a timely notification to the NRC regarding a therapeutic misadministration, and (2) the failure to obtain approval of the Radioisotope Committee for physicians to use licensed material. 3. Indiana & Michigan Electric Co. (D.C. Cook Nuclear Plant, Unit 2) payed the proposed penalty of $50,000 based on violations involving the inoperability of both Unit 2 independent emergency core cooling system subsystems while operating in Mode 1. (EA 87-13)

Other 2.206 Petition Mozart Ratner and Anthony Roisman filed a 2.206 petition on behalf of Vera English seeking enforcement action against the General Electric Company for the alleged improper dischcrge of Ms. English in viplation of Commission requirements. Specifically, they seek civil penalties of $40,635,000 plus

$37,500 per day until GE takes corrective action. In addition they seek an order from NRC modifying GE's license requiring corrective action, i.e.,

compensate Ms. English in the amount of $2,355,626.62, to repair the damage done or to have the license suspended.

(3) The Commission directed the staff in an earlier SRM to develop a plan for increasing the number of women and minorities in the upper grade levels and the SES. The staff's response was to propose a rotational program which l has yet to be implemented. The staff should immediately l identify rotational assignment positions for this program and report to the Commission when this has been completed.

Special emphasis should be given to expanding the scope of the rotational program under the current reorganization.

i Further, the Commission requests that the staff report to the Commission in detail on programs, other than rotational

programs, being used by other agencies to accomplish the 4 goal of increasing the representation of women and minorities in management positions.

l The staff should also identify opportunities under the reorganization to expand the upward mobility program.and to increase the number of women and minorities in first-line supervisory positions throughout the agency. Please present a plan within two months for accomplishing the expansion of the upward mobility and rotational programs.

(4) The Commission's hiring goals are being established based on the study of the workforce performed by Oak Ridge. It was the Commission's understanding before the Commission
