TECHNICAL REVIEW REPORT: RECENT OPERATIONAL EXPERIENCE TRENDS AT FERMI 2 l

The subject technical review is enclosed for your consideration. This study ,

was conducted to supply information for the latest regional management meetings. This study included cooperative efforts by AEOD contractors at both the Oak Ridge National Laboratory and the Idaho National Engineering Laboratory.

l It was found that the recent event frequency at Femi 2 has not been unusually high when compared to the rest of the industry or with plants from the same NSSS vendor. However, Femi 2 does show a . greater preponderance of events which are related to procedural deficiencies and personnel errors.

It is concluded that these problems are indicative of a willingness to begin, conduct and/or end a plant evolution without having quality procedures or quality procedure controls. Thus, the area of procedure development and control at Fermi 2 appears to require attention.
