FAILURE DESCRIPTION: THE REACH ROD IS JAMMED.

CORRECTIVE ACTION: REACH ROD FACE PLATE REMOVED. CLEANED BEARING HOUSING. REASSEMBLED REACH ROD. SATISFACTORY.

CORRECTIVE ACTION: EXISTING THERMOCOUPLE WOULD NOT HOLD OIL SEAL ON THRUST BEARING ALLOWING OIL TO LEAK THROUGH THERMOCOUPLE AND INTO CONDUIT. REMOVED DEFECTIVE THERMOCOUP LE. CALIBRATED AND INSTALLED NEW THERMOCOUPLE.
