. REPORT OF SELF-CITED VIOLATION FOR LICENSE #R-110, DOCKET #50-284 March 24,1995 Item A: Failure to maintain Nuclear instrument (NI) Channel #2 within the Limiting Condition of Operation set points as described in the facility Technical Specifications (Ref.10CFR50.36.c.(2) and Idaho State University Technical Specification Section 6.9.2.a.(2)).
- 1) Backoround of violation: On March 10,1995, while performing surveillance on NI Channel #2, the low level set point was discovered to be tripping at a value of 1.9 X 10'" amps. This is a value less than that specified in the facility's Technical Specifications 3.2.d (i.e. 23 X 10'" amps). The reactor had been operated several times since the last time the NI Channel #2 low level set point was verified on April 20, 1995. This is a violation of facility's Technical Specification Section 6.9.2.(a). The United State Nuclear Regulatory Commission (USNRC) was informed of this violation on the next working day (March 13,1995).
- 2) Reason for violation: The reason the NI Channel #2 low trip set point was found to be less than the prescribed value (as per the Technical Specification) for the LCO was due to instrument drift.
- 3) Corrective actions taken and results achieved:
The low trip set point for Ni Channel #2 was adjusted immediately to meet the LCO of the Technical Specifications. The surveillance for NI Channel #2 was completed successfully.
- 4) Additional corrective actions to be taken:
The Reactor Safety Committee will hold a meeting by June 30,1995 to approve a Technical Specification change request of Sections 3.2 and 6.9.2.a.(2). This change request will provide for remedial actions of adjusting the LCO's set points without having to file a Reportable Occurrence. Once the Technical Specification change is approved, this request will be forwarded to the USNRC for final approval.
- 5) Date when full comoliance will be achieved: Full compliance will be achieved when the USNRC approves the Technical Specification change request.
