Dear Sir:

This letter refers to the inspection conducted by Messrs. Bob Pate and Jack Garvin on April 10-14, 1978 as documented in your letter of June 14, 1978 of activities authorized by the Nuclear Regulatory Commission (NRC) Construction Permit Nos. CPPR-141, 142 and 143.

As a result of this inspection, a Notice of Violation was written

CPPR-141, 142 and 143 Notice of Violation Based on the appears that results of an NRC Inspection on April 10-14, 1978, one of your activities was not in full compliance it with the requirement of 10CFR50, Appendix B, Criterion VI, as im-plemented by the PSAR (Chapter 17).

Contrary to these requirements, on April 10, 1978, the NRC inspector observed that the change to the Marathon drawing on Change Notice VRC No. 666, dated March 15, 1978, was not distributed or used at the location where the work was being performed.

This is an infraction.'~Res oese The referenced Vendor Revision Change Notice (VRCN) was written against Marathon Steel's fabrication list (cut sheet). This re-vision had no effect on*the installation drawing 13-10407-Cll-F45-5 and, therefore, was not required at the location where the work was being performed. Consequently, in this instance this may not be a violation of the quality assurance program require-ments.

However, the drawing control. system used at Palo Verde depends on strict administrative controls and close attention by the Craft Superintendents and Field Engineers. In order to strengthen

'these controls, the following steps have been taken since the last NRC Inspection:

1. Procedure Change Notice (PCN) //7 to Work Plans Procedures (30P/P)'.0, Field Control of Design Documents'; was issued'n 'June 28, 1978 which states:

"The crafts shall be provided with DCN's, FCN's, VRCN's and FCR's as required to perform their work as determined by the Area Engineer (AE) or his designee."

2. Foremen and General Foremen have been verbally advised by the Area Field Engineers (AFE's) that it is their responsibility to request replace-ment of drawings and changes that are damaged or destroyed and to inform the AFE's of any known out-of-date revisions.
3. The Matrix which identifies the drawings that go to the respective crafts level is reviewed and revised on a monthly basis by the AE. New and revised drawings are distributed to the crafts as they are received or as determined by the AE.

The craft stick files are audited against the Matrix on a monthly basis by the Area Clerk to insure that the drawings are in accordance with the Matrix requirements.

4. During the Area Engineers/Lead Discipline Field .

Engineers meetings drawing'ontrol is discussed at a minimum of once a month, with emphasis placed on keeping drawing stick files current and to the latest Field Revision Log (FRL) ..and Daily Noti'fi-cation List (DNL), in accordance with the Matrix requirements as established by the AE's.

5. Design Document Control Center (DDCC) personnel perform audits at least monthly at Stations to the latest FRL and DNL in Maintained'rawing accordance with the particular area Matrix. Dis-crepancies which are found are corrected within 24 hours.
