Title | Licensee-Identified Violation |
Description | A licensee-identified violation for Unit 2 of TS 3.4.3 which states 10 of 11 SRVs shall be operable during Modes 1, 2 and 3. Contrary to this, on March 12, 2009, it was identified during bench testing that five safety relief valves failed to lift at the required technical specification (TS) setpoint. The cause was found to be corrosion induced bonding between the pilot disc and seating surface. This condition was documented in CR 2009102787. This finding is of very low safety significance because a previous evaluation performed by the licensee bounds this condition and RCS pressure would be maintained below the TS safety limit.
