Report Testing Error by a Health and*Huinan Services Certified Laboratory Summary of Issue: On February 13, 2018, a pre-employment drug screening sample was collected by Donald C. Cook Nuclear Plant (CNP) and submitted to Clinical Reference Laboratory (CRL) in Lenexa, Kansas for testing. On February 14, 2018, initial screening indicated that the sample specific gravity and creatinine were low and the sample was reported to CNP as negative dilute. Upon investigation, CRL discovered that the Screening Technician did not load the sample on the refractometer consistent with Standard Operating Procedures (SOPs). Due to the misleading of the sample, an incorrect specific gravity value was obtained which resulted in the sample being reported as negative dilute. Subsequently, CRL personnel tested a new aliquot from the primary sample consistent with the applicable SOP. This testing result showed the sample was not dilute. On February 16, 2018, a corrected report was issued to CNP indicating that the sample was negative.

On February 21, 2018, CRL reported the results of the investigation into this error to CNP. CRL has performed the corrective actions detailed below to prevent recurrence.

Corrective Actions: 1. The CRL Screening Department received retraining on the applicable SOP for loading the refractometer.
