-117.1, it appears that cooling to each diesel generator must be manually aligned, therefore the selected answer choice is not fully correct. I understand that the pump starts automatically, but there appear s to be valves that must be manually aligned. The question also does not seem to be worded to ask for a reason, therefore is not a good K/A match. I have a very easy fix that we can discuss.

4/29/16 - Reviewed the revised question. The question stem asks whether fire service water will be initiated manually or automatically in accordance with the AOP. The AOP directs that it be done manually, but the conditions listed would have caused it to be done automatically. Need to tighten up the question.

Regarding the SRO only justification for question #76, please explain why this is not SRO only.

4/29/16 - Revised question is satisfactory.

2 H 3 X N E K/A 008AA1.06 Question choices combined with the answer choices are long and difficult to read. Could easily split this up into two questions. Need to ask both questions in accordance with the applicable procedure. Need to discuss this question.

4/29/16 - Revised question is satisfactory. 3 H 3 X B E K/A 009EK3.23 Is it possible for a SBLOCA to cause a 13 psig pressure rise in containment in one minute?

4/29/16 - Revised question is satisfactory.

4 F 3 X M E K/A 011EA2.08 Based on the procedure step, the stem question should read "the CCW train in the active loop," vice "the CCW train with the active loop." 4/29/16 - Revised question is satisfactory.

5 F 3 M S K/A 015AA2.09 Question is Satisfactory.

6 H 3 N S K/A 022AK1.03 Question is Satisfactory.

Partial Job-Link Minutia #/ units Back-ward Q= K/A SRO Only 7 F 3 N E K/A 025AA1.11 Are the conditions given operationally valid? If you are below the RWST level where automatic switchover should have automatically occurred, but also below the sump level where ECCS pump suction requirements are not met then it seems to me that this is a very poor plant design or water from the RWST went someplace other than the sump which would lead to choice A also being potentially correct. Verified that conditions would lead to EOP

-2.5 entry.

Question is satisfactory.

8 H 3 B S K/A 026AG2.4.49 Question is Satisfactory.

9 H 2 X X N U K/A 029EK2.06 Since all of the bullets in step 1 of EOP

-1.0 are not met for verifying the reactor tripped, could an applicant argue that the reactor is not tripped? Need to specify in the initial conditions that all or both reactor trip breakers are closed.

Choice B is not a plausible distractor when contrasted with choice A.

Choices D is not plausible. Question is unsatisfactory due to two non-plausible distractors.

5/3/16 - Revised question is satisfactory.

10 H 3 X X N E K/A 038EK1.03 Discussion on page 22 of 34 in the EOP

-4.0 training handout seem s to describe choice D as another correct answer.

4/29/16 - Revised question is satisfactory.

11 F 3 X X B E K/A 040AG2.1.31 Could have XCP 615, 1

-2, RCS TAVG LO in alarm but not yet have a feedwater isolation (1/3 for alarm vice 2/3 for feedwater isolation). I do not believe that both B and C are both plausible distractors since they are at the same setpoint for the same parameter.

4/29/16 - Reviewed revised question. What makes choice A plausible? What caused the reactor trip? Need to further discuss.

5/2/16 - Based on discussion with the licensee, the revised question is satisfactory.

Q# 1. LOK (F/H) 2. LOD (1-5) 3. Psychometric Flaws

Most issues identified during the sample review were addressed however distractors A and B are still basically the same thing stated with different words.

5/4/2016 - Revised question is satisfactory.

13 H 3 X N E K/A 056AA2.32 The difference between 557 and 559 degrees F is minutia.

4/29/16 - Revised question is satisfactory.

14 H 3 X X  ? N U/E K/A 057AG2.1.19 Need to discuss this question. The first part of the question does not seem to relate to the loss of vital 120VAC power which would mean that the K/A was not matched and would make the question unsatisfactory. Do you have any breakers that are controlled by AC power for plausibility of the distractor? Are the applicants expected to know by memory what APN

-5901 is along with its loads?

K/A 057G.2.1.30 5/5/16 - Revised question is satisfactory.

What is the significance of reading step 1 of AOP

-117.1? The title of AOP

-117.1 is not TOTAL LOSS OF SERVICE WATER.

Based on step 15 of AOP

-117.1, it appears that cooling to each diesel generator must be manually aligned, therefore the selected answer choice is not fully correct. I understand that the pump starts automatically, but there appear s to be valves that must be manually aligned. The question also does not seem to be worded to ask for a reason, therefore is not a good K/A match. I have a very easy fix that we can discuss.

Question is unsatisfactory due to incorrect answer and K/A mismatch.

The AOP directs that it be done manually, but the conditions listed would have caused it to be done automatically. Need to tighten up the question.

Instructions

[Refer to Section D of ES-401 and Appendix B for additional information regarding each of the following concepts.]

1. Enter the level of knowledge (LOK) of each question as either (F)undamental or (H)igher cognitive level.
2. Enter the level of difficulty (LOD) of each question using a 1 - 5 (easy - difficult) rating scale (questions in the 2 - 4 range are acceptable). 3. Check the appropriate box if a psychometric flaw is identified:

The stem lacks sufficient focus to elicit the correct answer (e.g., unclear intent, more information is needed, or too much needless information).

The stem or distractors contain cues (i.e., clues, specific determiners, phrasing, length, etc).

The answer choices are a collection of unrelated true/false statements.

The distractors are not credible; single implausible distractors should be repaired, more than one is unacceptable.

One or more distractors is (are) partially correct (e.g., if the applicant can make unstated assumptions that are not contradicted by stem).

4. Check the appropriate box if a job content error is identified:

The question is not linked to the job requirements (i.e., the question has a valid K/A but, as written, is not operational in content). The question requires the recall of knowledge that is too specific for the closed reference test mode (i.e., it is not required to be known from memory). The question contains data with an unrealistic level of accuracy or inconsistent units (e.g., panel meter in percent with question in gallons). The question requires reverse logic or application compared to the job requirements.

5. Check questions that are sampled for conformance with the approved K/A and those that are designated SRO

-only (K/A and license level mismatches are unacceptable).

6. Enter question source: (B)ank, (M)odified, or (N)ew. Check that (M)odified questions meet criteria of ES

-401 Section D.2.f.

7. Based on the reviewer's judgment, is the question as written (U)nsatisfactory (requiring repair or replacement), in need of (E)ditorial enhancement, or (S)atisfactory?
8. At a minimum, explain any "U" ratings (e.g., how the Appendix B psychometric attributes are not being met).
