' surveys as (1) may be necessary for the licensee to conply with regulations in,this part and (2) are reasonable under the circumstances to.
evaluate the extent of radiation hazards that may be present.
Contrary to the above, from January 1, 1988 to February 28, 1989, licensee survey's were. inadequate to evaluate the extent of radiation hazards present in that:
1)
Routine' quarterly radiation exposure surveys conducted in the reactor cell restricted araa failed to ' adequately evaluate the elevated dose rates and radiaticn fields associated with the center and west vertical reactor beam ports.
.For each quarterly survey, records indicated measurements were made only for a single location approximately one meter'above the reactor shield surface over the center vertical beam port.
2)
Routine quarterly radiation exposure surveys conducted within and outside of the reactor cell restricted area failed to identify a collimated radiation beam resultinc in radiation dose rates exceeding 10 CFR 20.105(b)(1) limits of two millirem per hour (mR/hr) outside the west wall of the reactor cell restricted area.
This is a Severity Level IV violation (Supplement IV).
