Purpose (Statement of Problem):

The seal weld on the bonnet leak-off plug was leaking and the resulting steam cutting damaged the plug and bonnet.

Narrative Summary: (Brief Description of Work Perfornied)

The piping plug was removed and replaced with a new plug in accordance with FCN S2242H.

The head of the plug was ground off and a plug weld was made which fully covered the pipe plug.

The completed weld was MT inspected and a system leakane test was performed in conjunction with a VT-2 visual examination.

During the performance of M.0. 86051232 the 1/2" NPT drain connection on the bonnet was inadvertently broken off. The drain connection internal threads were damaged in the process. Additionally, an inspection of the stuffing box revealed severe erosion and steam cutting. Both of the above conditions were documented on

Narrative Summary:

1 note: The following repair work was performed at the valve l

vendor's (Control Components Inc.) facility.

The 1/2" NPT bonnet drain connection was bored oversize. A PT examination of the bored surface was performed and the hole

]

was then tapped to 3/4" NPT.

2.

The stuffing box area of the bonnet was bored out to accept a bushing. A PT examination was performed on the machined surfaces.

3.

After the bushing material was verified as meeting ASME III, class 2, 1974 edition, Summer 1974 addenda, the material was machined to the dimensions specified on the NCR for the bush-ing. A PT examination was performed on the outer surfaces after machining.

4.

The bushing was installed in the bonnet and welded in place.

A PT examination was performed on the inner veld and an MT examination was performed on the outer weld. The bonnet as-sembly was then postweld heat-treated.

The installed bushing was then bored to the final size spec-ified for the stuffing box. The weld reinforcements were also l

machin.ed flush. A PT examination was performed on final bore and machined weld reinforcements.

6.

A 3250 psig hydrotest and VT-2 Inspection was performed on the repaired bonnet assembly.

7.

A Receiving' Inspection was performed on the repaired bonnet by SCE after shipment from CCI (reference RS0-0599-87).

8.

The bonnet was reinstalled on the valve under M.0. 86051232 and Traveler S03-p98-87.
