This letter and the enclosed NRC Form-591 refer to the safety inspection conducted on October 7,1998, of activities authorize.1 under your NRC Radioactive Materials License No. 50-23219-01. Based on the results of this inspection, the NRC has determined that one violation of NRC requirements has occurred. This violation is cited in the enclosed NRC Form-591.
Enclosed is the NRC license amendment which you requeste Please review the enclosed document carefully and be sure that you understand all condition If there are any errors or questions, please notify us so that we can provide appropriate corrections and answer ,

Your license has been changed to be consistent with current NRC polic I 10 CFR Part 35 was amended effective April 1, 1987. A copy is enclosed for    i your reference. Leak testing requirements are stated in 10 CFR 35.5 ;

Pursuant to the provisions of 10 CFR 2.201, you are required, within 30 days of the above date, to sign and date the enclosed NRC Fonn-591, indicating that appropriate action will be taken to correct the violation described on the form. After printing your name, signing arid dating both copies of the NRC Form-591, please keep one copy and return the remaining copy to me at the address above.
