all conditions. If there are any errors or questions, please notify us so
that we can provide appropriate corrections and answers.
As it was not possible, from the information submitted, to determine whether
the orthopedic surgeons Ifsted in attachments to your letters have completed
all of the training necessary for Lixiscope users, they were not listed on
your license at this time. All uses of the Lixiscope must be conducted in
the presence of Drs. Holloway or Paton.

It should also be noted that letters from the administrator of each hospital
ifated in Condition 10. should be maintained on flie in your office to
verify that Lixiscope use at each institution has been approved by the
hospital's management.
